Information and Forms for Students with Health Concerns

School Nurses, along with parents and Health Care Providers of students with specific health concerns, create an Emergency Medical Plan (EMP) to ensure the student's safety while at school.

EMPs are specific forms tailored for each student's need.

Accompanying forms that support the EMP must be completed when students are newly diagnosed, have changes in their health or care plan, and at the beginning of each new school year. Click here to view forms.
